CPT 97530 Therapeutic Activities: When to Use It, How to Document It, and Avoiding Denials

CPT 97530 often pays a bit more than therapeutic exercise, which is exactly why payers watch it closely. Used correctly, it reflects the functional work your clinicians do every day.

What CPT 97530 covers

97530 is therapeutic activities: direct, one-on-one, dynamic activities to improve functional performance. It is a timed code billed in 15-minute units.

Think of tasks that look like real life: lifting and carrying, bed mobility, transfers, reaching, balance during a functional task, or pediatric play-based activities aimed at a functional goal.

The one-on-one requirement

97530 requires direct, one-on-one contact. If the clinician is splitting attention between patients, the time doesn't qualify as 97530. Medicare treats that as group therapy (97150) when two or more patients are treated at the same time doing the same or similar activities.

What to document

  • The functional task — not just the body part.
  • How the activity connects to a functional goal in the plan of care (for example, "to return to lifting 30 lb at work").
  • The skilled input: cueing, grading, safety training, or progression.
  • Minutes spent on 97530 separate from other timed codes.

NCCI pairs to watch

Medicare's NCCI edits bundle certain codes when billed on the same day. 97530 is paired with codes such as the PT and OT evaluation codes and 97140 manual therapy. These pairs can be billed together only when the services are truly separate and distinct, using modifier 59 (or an X modifier) with documentation to support it. Always check the current quarterly NCCI tables — pairs change.

Why 97530 gets denied

  • Notes that describe exercise (sets and reps) rather than a functional task.
  • Missing modifier 59 on a bundled pair, or using it without separate documentation.
  • Time not clearly separated from 97110 or 97112.
  • Group or concurrent treatment billed as 97530.

Quick answers

What is the difference between 97110 and 97530?

97110 targets a specific impairment like strength or range of motion. 97530 uses dynamic, functional activities to improve performance in real-life tasks.

Does 97530 need modifier 59?

Only when billed with a code it is paired with in the NCCI edits, such as 97140 or an evaluation code, and the services were separate and distinct.

Can speech-language pathologists bill 97530?

97530 is generally billed by physical and occupational therapists. SLPs typically use speech-specific treatment codes such as the 2027 codes 92654–92663. Check payer policy for exceptions.
