How to use it

Five habits that make the template work

Quote the worst case first. If the family is comfortable with the maximum exposure, the best case is a pleasant surprise — never the reverse.
Put your fees in writing with the estimate. A quoted price the family can't see in print is not a quote; it's a guess.
Pair the estimate with a card on file. The acknowledgment signature is what makes your payment policy enforceable and fair.
Re-verify benefits when an authorization renews. Benefits change in January and whenever the family's employer renews the plan.
Keep the paper trail. Save the signed estimate, the verification reference numbers, and every email — that's what wins disputed denials.
