The basic timeline
Medicare expects the initial plan of care to be certified by a physician or qualified practitioner, generally within 30 days of the initial evaluation. Recertification is needed when the plan changes significantly and at least every 90 days.
Delayed certification
Late certifications can still be accepted when the practice documents good-faith efforts to obtain them. Keep a record of every request and follow-up.
